From start to finish – unsurpassed patient care


Start

To order a study, simply complete and fax the sleep study order form found in the Sleep Resources folder which was brought to your office. If you do not have one, please call our office and an order form will be faxed immediately. Please include office visit notes when they become available.

Appropriate procedure codes:
95810 – Diagnostic Polysomnography (for initial sleep study)
95811 – Polysomnography - CPAP Titration
95865 – Multiple Sleep Latency Test

Scheduling

After receiving your order form, we will contact the patient to schedule the study.

Education

The education process begins at the time a patient is scheduled for a sleep study. This is vital to patient comfort and ensures his or her understanding of the process and what to expect from the test. It also provides our staff with the necessary information that is vital in anticipating a patient’s individual needs.

During a thorough patient education session regarding the sleep study process, each patient receives a packet containing:

  • A booklet so they know exactly where to come and what to bring
  • A brochure on sleep apnea and other individualized sleep information
  • A video to show them what a sleep test looks like as well as provide more information on sleep apnea and CPAP
  • A sleep assessment to complete
  • A sleep diary to complete

Testing

Sleep Resources, Inc. combines state-of-the-art equipment with more than 60 years of combined professional experience in the field to provide the best assessment possible. Data is analyzed and reviewed by registered polysomnographic technologists and read by physicians board certified in sleep medicine.

Results

Results are returned to the referring physician within ten days and immediate notification is done in severe cases.

Follow up

In order to ensure continuity of care:
  1. Patients are always informed that they should make or keep a follow-up appointment with the ordering physician
  2. Each patient is called after their study is read by the physician to:
    1. ensure they have a follow-up appointment with the ordering physician or sleep specialist
    2. see if they have any questions or concerns about their sleep study
    3. make sure they have been contacted about getting started on CPAP therapy when it has been ordered

Patients with significant sleep apnea who express a wish to begin CPAP therapy are contacted as soon as statistics are available to allow insurance pre-authorization.
